Oracle Forms 12c. could not open xml form

I can not open a form file saved as xml document (using conversion tool in oracle forms 12c). opening form_fmb.xml document come into FRM-10043 error.

I also tried to convert a fmb file into xml, using both Forms conversion tools and frmf2xml script; but none of them worked.

so, I think conversion tool is not properly installed. how can I get it work?
