Query on WO corrupt media object text attachment

Good Day,

I understand that there is a known issue documented as below where a text attachment defaults on all new work orders (and the fix is to delete the corrupt F00165 record). My question is whether anyone has been able to figure out on why it happens in the first place as it has become a recurring issue for us and we are unable to find any pattern to it.

Your inputs and assistance is much appreciated in this regard!
