Voluntary Deduction being taken out in both Regular Payroll and Separate run

An employee has a grossup element that is processed in a separate run. Recently they had a voluntary deduction element entry created and this is being deducted in both the regular and separate payroll runs.

Just wondering if anyone has encountered this before and any ideas where I can begin looking to identify the isse?
