Isupplier attachment not found

We are using oracle R12 SLM ISupplier, in Prospective Supplier Registrations, when the supplier still pending for approval after opening the attachment for one time then it despairs and the filed show nothing as the below screenshot.

As I enter as below:

navigator > BABD Supplier Management Administrator >supplier home >Prospective Supplier Registrations > click on supplier review.
