P08EFT Change Reason form displayed twice following a Salary change

When updating the Employee Master, and making a change on the P0801CMP/W0801CMPB form (Basic Compensation), upon clicking the OK button, the P08EFT Change Reason form is displayed. We enter the Change Effective Date and Reason, click OK, and the same form is displayed over again and we must enter a second time around the Change Reason before we are taken back to the P0801 employee list. I have checked for bugs on Oracle but did not find any.
