Queries using supplemental data table

We are trying to build a query that includes our data from the supplemental data table. We use several SD fields to capture data. But because there are multiple SD fields we want to include the query returns a new row for each field, thus giving us multiple rows of the same record. Is there a way to create the query so that all the supplemental data rows pulling in, but the records shows only once?
