How to know which application form is used in service request

Hello,

In P98220U, is there a way to knows which application is used in Form Requests ?

Example, I've created a Form Requests in Orchestrator, but if I go in P98220U, I can find the Form Requests with the name of it, but the Object Name and Form Name still blanc. Are those informations stored in other table or xml file ?
