Wrong user on PO activity history

Good day,

I found an error in the Purchasing module, specifically in the PO activity history section. In the "Provide Information" part, the user listed is incorrect; it should not be this user who performed the action, but someone else.

Herewith I attach the screenshot , it is supposed to be Fitrotul who provide the Information, but on Oracle "performed by" column is the one who request the information.
