Audit trail for User responsibilities start date , end date

I want to create a audit trail for capturing details of assigning users responsibilities and end dating user responsibilities, so that we can capture who assigned

responsibility to a particular user and also capture when and who end dated the responsibilities . Is there a way to achieve this and would also like to know how to generate audit report for this .
