moving an old EBS 12.1.3 environment to a new set of servers

One prem EBS setup (db tier and single app tier on sperate servers) and I have run lots of rapid clones 12.1.3 and now 12.2 and it is most always cloning prod to a test/dev instance. In this case I just want to move an old 12.1 installation from one set of servers to another and I am not sure what all steps I need to perform to make it happen. Like I know the hostname portion will need to change and then I either also change the CDB/PDB names or keep them the same if easier as
