Urgent and High Priority - Getting oracle.xml.parser.v2.XMLDOMException

When the ADF form i.e. .JSPX page is getting submitted (where the ADF page is created suing the Human task and used within the BPM Interactive activity ) then , the following exception is coming up with no hint mentioned whasoever about which XML element is having the issue in parsing . We tried to find the details from Oracle EM and Oracle SOA DB however could not get any useful information . We have a Prodution release coming up and this has become a critical roadblocker for us.Pls help in figuring out what is causing the issue
