Auto-Generating Not Auto-populating

Hello:

I wanted to find out if there is an issue/bug with records not auto-generating. For one business process RFI ⇒ PCO if certain conditions are met the RFI will auto generate a PCO. There were no changes to this process and not it "hit or miss" with autogenerating. I checked and re-checked the conditions, tried saving the RFI to see if it would auto-gen (this used to work before), going to the PCO module to create a PCO (this used to work with auto populating the information from the RFI) to no avail.
