An issue on Self-service Fluid Life Event Page

Hello Experts,

We are experiencing a very odd issue on Fluid Life Event self-service page, it happens intermittently and there is no pattern. Sometimes when employee initiates a life event on Life Event Fluid page, it does not save correctly, specifically in table PS_W3EB_LE_STATE92. Because of missing data in this table, next time when employee goes to Life Event Fluid page, it closes immediately.

I have tried to reproduce in a non-prod instance (copy of production prior to employee initiated life event in prod) for the same employees but could not reproduce it. I have tried to reverse engineer the code but could not find any reason why data will not be inserted in PS_W3EB_LE_STATE92.
