Has anyone had issues with the event registry AssignmentTermination after PUM 52/Tools 8.61.12?

We have offboarding running currently on PUM 49/Tools 8.59.15 and everything works correctly. However, we are updating to PUM 52/Tools 8.61.12 and having issues with the event being created for employees who have had multiple records.

Employe ABC with Employee Record 0 and 1.
Employee terminates record 0 and event is successfully created
Employe terminates record 1 - No event is created and no offboarding AG is created.

Has anyone else seen this or know how to troubleshoot?
