out of place patching for grid and DB home

Is there documentation available from oracle for out of place patching for grid and DB homes ?

Plan is to download and layout most current CPU bundle and apply MRP and 555 patches to same and switch to using new home .

Additional question for DB home, is it advisable to create clone of existing home or layout new home ?

Please provide any Support docs if available.
