Federal/State Taxable Gross not calculating accurately

Has anyone else experienced issues with Federal/State Taxable Gross not calculating correctly? It seems to be randomly impacting individuals who have supplemental earnings and before-tax deductions. We've encountered individual paychecks (not everyone all the time) with Federal/State Taxable Gross calculated as follows:

All Earnings plus SUP Special Accumulator earnings

instead of:

All Earnings less before-tax deductions plus taxable deductions

When it does calculate the correct Taxable Gross, the tax is calculated correctly by prorating the before-tax deductions based on the amount of supplemental earnings.
