Title: SL4000 – Dual LOH Failure / Rebuild Issue

in Inventory Management - EBS (MOSC) Question

Dear Oracle community ;)

We are troubleshooting an Oracle StorageTek SL4000 with the following LOH status:

  • Storage(library,1): Online / Operative
  • Storage(library,2): Offline / Failed
  • Storage(library,3): Offline / Failed

Firmware: 1.1.1.112.40469
Platform: 1.0.0.17.40005

The library works in degraded mode with the original hardware.

When we replace one failed LOH, the new card is detected and enters Rebuilding, but shortly afterwards the library services become unavailable. Reinstalling the original failed LOH restores the library to the previous degraded state.

We reviewed KB192790, which describes the standard LOH replacement process, and also found Oracle release notes stating that there is “no easy recovery if 2 Library Controller LOH failures occur concurrently.”
