Advanced Email Template not printing Remittance form

I'm using the Advanced PDF Invoice form + the standard remittance form as a test.  Printing Preferences set to "PRINT REMITTANCE FORM WITH INVOICES & STATEMENTS", but remittance form isn't printing.  Any ideas? 

FYI - I asked NetSuite support and they logged an enhancement, saying it's not supported, but that is hard to believe seeing as how the documentation on suiteanswers indicates that it is:
