Lost field in Assembly build custom form

on the Assembly build custom form there is a standard filed expirationdate, once I keyin the expiration date and saved the form, the expiration field no longer there on the record. Even when customized, there is a option to include field but it does not show up. How to bring it back as it is marked to show and even mandatory but is no where to be found.

Any suggestions would me much appreciated!
