Worfklow Email not delivered to original recipient

I have workflow for Requisitions, Purchase Orders and Vendor Bills. When a Requisition, Purchase Order or Vendor Bill needs approval, the Approver does not receive a notification email but the creator receives an email with the subject showing that the email was originally sent to the Approver. However, the approver did not receive the email. Sample workflow action parameters for the PO is attached.
