SuiteApprovals 6.00.5 Throws Error Upon Sales Order Creation

I have installed the current version (6.00.5) of SuiteApprovals on my sandbox environment.

After setting up a simple department approver list and an approval rule, when I try to save the sales order, an error message returns:
