CSV Import > Field Mapping is blank

It hasn't mapped any fields and clicking the + icon on my fields doesn't open anything. Is it not recognizing the fields?

Tried logging out and in, clearing cache, refreshing the page. Seems to be the CSV

Changed the CSV to the below and now it's working. I'm not sure what the deal was.
