How to create a VRMA PDF?

Can somebody provide me information on how to make a just PDF document, not an advanced PDF.

What I want to do is to create a PDF to be able to print the VRMA.

The PDF will contain, the fields marked in red.


That PDF I wanted to use when the user click on the PDF after they create the VRMA.
