Why is zero dollar Purchase Order Pending Approval

PO15145 is a zero dollar PO that has been approved. However, the status is still Pending Approval. As a result, no transactions can be processed against this PO.

What is causing this? Although it's happening sporadically, it does stop business. See attached document with screen shots.
