Netsuite Advanced Manufacturing - Duplicating Issue Transaction

When we are completing a work order issue in Advanced Manufacturing, it is triggering the work order issue transaction twice as two identical transactions with unique ID's with the same create date and time in the system record. What could be causing this duplication?
