Allow user to select Subsidiary when submitting an Online Customer Form

Hi all!

Apologies if this question was already asked, but I could not find any resource on this. I have a Online Customer Form that is embedded on our website. Currently, the form works as it should and is pulling all information I need such as: Name, Email, Company Name, Phone, etc.

The issue I am having however, is I want the customer to be able to select a Subsidiary. We have 3 different subsidiaries that create different types of products. Ideally, the user can select a drop-down and select which subsidiary they would like to request a quote from. Currently, I am trying to display this using the <NLSUBSIDIARY> tag but it is not showing up.
