In CSV transaction imports has anyone had trouble updating and adding transactions at the same time?

[Edit - cleaned up my explanation a bit]

The transactions CSV import is not creating separate new transactions when using "add or update." I have a CSV with 5 transactions (purchase orders), 3 are being updated (and have their internal IDs and line numbers identified in the CSV), and 2 are new transactions to be created (no internal ID is given, but I've given them two unique document names and two unique external ids). No matter how many different things I try, the two new transactions are being created as a single transaction. Even though they have different document names, different external IDs, and different locations
