Table formatting applied in a Microsoft Word RTF document template is not properly applied by the document generation service, used in Oracle Policy Automation.
The attached document show quite a lot of example.

I do not have time to test every scenario that you have. Suffice to say, I have learnt that you can not simply copy an already formatted table and then make changes. You need to start from a fresh simple table each time.
That said, it looks like you have tried that with some of your scenarios and it is not rendering correctly.
Id recommend logging an SR with BIP support to get some more inout form the development team
