"Lost data" during performance reviews

We receive occasional comments concerning the performance review not saving data. We never have been able to pinpoint a cause, nor even precise information as to when and where the loss occurs.

We are using Taleo Performance and currently on 14B.  We receive occasional comments concerning the performance review not saving data.  These comments do not seem to be version related.  They have occurred throughout the years on many versions.  We never have been able to pinpoint a cause, nor even precise information as to when and where (on the form)  the loss occurs.   Often the comments are like, "I don’t know exactly when it happened – I did it several months ago and when I went in to finalize it was blank."
