How to capture additional info at employee level

We wanted to capture some extra info shown in below image (Certification details that employee has earned) against employee record.
Certification Name 1 | Start Date | End Date | |||
Certification Name 2 | Start Date | End Date | |||
Certification Name 3 | Start Date | End Date | |||
Certification Name 4 | Start Date | End Date | |||
Certification Name 5 | Start Date | End Date |
What is the best way/approach to achieve above requirement?
