Performance Evaluation - Send For Review Process - not getting initiated upon clicking tthe Begin Button in the task

Hi ,


For few employees, First Approval Task is not getting routed to the reviewer and instead of that the Performance Process is moving back to the Previous Phase

Any Idea , how to trouble shoot this issue.
