Is it possible to use the questionnaire outside of performance module?

We are looking for something to make use of the questionnaire to store and display data for the employees. Yes, we have the document of records, but we are not quite satisfied by the way it comes up. Questionnaire was a best fit and we wanted to use it to capture user's response
