Duplicate Performance Document for an Emploee in Update Performance Tasks

Hi,
When we search for an employee in "Update Performance Tasks", 2 entries appears for the same employee and same performance task - one with Employee and another with Manager. Searching for PD in other tasks such as "Search for My OrganizationPerformance Task", "Cancel Performance Tasks" fetches only one value.
Please let me know if anyone has faced this earlier.
