Hi, I have a requirement to update our feedback template to include section for rating competencies, identify three areas of strengths and development areas.

Hi,

I'm looking to update the feedback template (which currently is a questionnaire with four questions) to now include a section to assess competencies and provide ratings, identify 3 areas of strengths and development areas (open ended text box).

Is this even possible especially the competency ratings section? Any suggestions or ideas on this will be very much appreciated.
