Goal approvals stuck in pending approval status / goal approval error

Employees are submitting goal plans which are sending for approval and then getting stuck in a "waiting for approval" state

We're experiencing errors with the redesigned goals user experience and updated approval process. Employees are finding when they sometimes submit a goal, it then tells them the goal is waiting for approval, but the manager has no way to approve and no link to see what needs to be approved. Admins are also unable to force approval in the transaction console OR in the pending goal plans task. Sometimes also happens when managers "request for more info" or when employees try to cancel a goal, but we can't pinpoint the issue.
