Error when submitting Goal Plan for Approval

Certain employee keeps getting "Error" message when Goal Plan is submitted for approval

Content

We have one employee that has received error messages when her goal plan is submitted up for approval.

We confirmed that she has opened the email via her computer and not on a cellphone.

Has anyone else encountered this issue?
