newly added test Goal rejection locked previously approved goals

Summary

newly added test goal to already approved list of goals was rejected as manager (for testing purpose) but it now locked information on all other already approved goals

While preparing a user guide, we added a test goal to already approved list of goals and sent for approval.  As manager, we rejected the goal but now goals that are already approved, that information is locked for both employee & manager.  See attached screenshot.  Has anyone run into any similar issue & how can I fix this?  

Note: I tried checking the Transaction Console to search this employee and terminate the process but I cannot find the employee either.
