Performance Goals Disappearing After Submitting for Approval

After some employees submit a performance goal for approval, the goal disappears and we are unable to locate it in the HRG_GOALS table or with any pending status queries

When forcing approval on these transactions, the goal plan unlocks with no goals approved and no record of previously submitted goals for this review period. The manager also has no option to approve.

We've run several queries and found no data for these goals and nothing in any tables showing them as pending. We used a transaction data cache query given in our SR and found the cached Goal IDs only, but when trying to find the corresponding goal info (name, description, comments) nothing exists. This is not limited to submission date, it has happened for people who submitted months ago or just this week.
