Checklist Tasks with Dropdown Option and then eSignature

We have a task that we would like employees to go through the document and then an option to Agree or Disagree (may be a dropdown) and then do the eSignature to complete the task.
With eSignature we can only capture Name and Email Address, however, it does not allow to add one more field as I agree or Disagree. Is there any other task we can use to capture this?
