Mass Goal Upload HSDL

Can anyone share mass Goal upload HSDL template? I have a template but there're some "ID" column such as Goal Plan ID/Assignment ID, which aren't friendly to enduser for upload, I tried to remove these fields but failed.  any sample templates are helpful to me.  Thx a lot.
