Automatically send employees a questionnaire every year

We have a business need to send a questionnaire to an employee on the anniversary of becoming a member of staff. I know I can trigger a checklist with a questionnaire linked to an action but there is no specific action to link this to.

Is there a way to send a questionnaire to a member of staff each year and how would could this be implemented and automated?

Any ideas welcome!
