Journey Questionnaire Shows Contact Info but does not pull in Phone

Summary:

We created a Questionnaire to use for COVID Vaccination tracking.

At the top of the Questionnaire from within a Journey task, a box shows stating "Contact Info". When we drop the arrow to unhide, the name and email of the shows but the Phone is not appearing.

Content (required):

As stated above - the top of the Questionnaire from within a Journey task, shows the Contact Info of the Journey assignor. When we drop the arrow to unhide, the name and email of the Journey creator shows but the phone number does not.

Can we either:

Hide the contact info
