Questionnaire links showing broken

Summary: Links in questionnaire show broken and aren't functional

Content (required): Hello...we created a questionnaire and for a couple of sections, we added a PDF attachment that the users should be able to open and read. However, on the document, these icons appear broken so the links are not working...are we only able to add images? or should we be able to add a PDF file?
