Flat Rate per Person Enrolled

Summary:

Client requirement is to add a flat rate per dependent enrolled to the EMP rate.

An Example: My family has Me + Spouse + 3 kids

My Life Insurance ends up being $4.58 and the per person rate is $3.455

The total Life Insurance premium is $18.40

Anyway to do this without a Fast Formula?
