Unable to delete redundant approval notifications resulting in Error

We are facing this error in PROD ( post go live) and we have been able to replicate it in lower environments , with no resolution-- hence reaching out .

Scenario -

1-Employee submits a Goal -Goal Plan for approval

2-Manager does not act on it .

3-Employee withdraws the Goal .

4- Manager now proceeds to "Approve"/ " Reject " ( unaware of the withdrawal / just timing issue ) . Clicking on any option results in an error

5- Manager has been informed its a redundant notification and needs no action , but is unable to remove it from the list

6- Show All notifications and Dismiss All - works on clearing FYI , but not unactioned approvals
