DFF required in evaluation section page for performance goals

Content (required):

We need to create one custom field in the performance document's evaluation topic section. Where along with the employee or manager rating the custom field would be used to provide one additional set of rating for one section item. Is the requirement feasible. I've also added the screenshot of the screen where dff is required.
