Can the Questionnaire be made visible to only the manager but included in employee print out?

Summary:

Hi peers,

Our client wants to include a questionnaire in the performance document only for the Manager to respond to. However, they want this included when the employee prints out the document after the evaluation process.

The problem is when I include the employee role in the questionnaire section as shown below,

the employee also sees the questionnaire while doing self-evaluation.

However, when I do not include the Employee role in the questionnaire section as shown below.

the employee also can't see the section in the printed-out performance document after the evaluation.

So, in summary, our client wants employees
