Performance Review Period but no Assessment

Summary:

Employees can see the Review Period within ME - Career and Performance - Performance

But cannot see the 4 questions associated with the Performance Review Plan

I have gone through the steps on our SDD on how to create each field from Set Goals to Performance Document, Review Periods, Performance Eligibility Etc and ran all the BULK ESS updates required and all say they have passed.

The only thing im failing to manage to do is the "Move Performance Review to Self-Assessment Task" as when i chose the option relevant to this year the actions button is still greyed out, so im assuming thats the issue.
