How to have a performance template with both the goals' assignment and competencies?

Hello,

Since we need a step to freeze the performance document once it is displayed and accepted by the employee, we would like to know if it is possible to have a performance template in which we have both the goals' assignment and the inclusion of competencies, without including the step of self-assessment of these by the employee?
