Goal transaction went to failed state with error BPEL_FAULT, even though the goal is approved.

Summary: Employee who submitted the goal has received the email of failed state with error BPEL_FAULT, after checking the same transaction we found the transaction got approved. Upon analysis in the Transaction History at on e point the transaction shows error BPEL_FAULT, but after that it got approved and went into completed state.

We have received same issue from couple of employees who have received the failed status goal transaction, but their goals transaction got approved by their managers.
